Healthy Children and Youth (HCY) program is a Medicaid based program that provides home healthcare for youth with medical and behavioral problems. Priority One Healthcare Services provides a care plan that meets your individual need. Our caregivers are highly qualified and trained for the needs of our behavioral youth.
CLIENT QUALIFICATION
- Receive Medicaid
- Have a physical disability
- Able to self-direct
CAREGIVER QUALIFICATION
- At least 18 years old
- Pass background checks
- Able to read and write
Private Duty Nursing (PDN)
At Priority One Healthcare Services, we provide high-quality Private Duty Nursing (PDN) designed for individuals who require continuous, skilled medical care in the comfort of their home.
Our licensed nurses (RNs and LPNs) deliver one-on-one care tailored to each client’s unique medical needs, ensuring safety, stability, and peace of mind for families. PDN services are ideal for individuals with complex or chronic conditions that require ongoing monitoring and hands-on nursing support beyond traditional home health visits.
Care is provided in scheduled shifts and guided by a physician-approved plan of care, allowing our clients to remain at home while receiving hospital-level attention in a familiar environment.

Key eligibility (especially important)
For children in Missouri:
- Must be on MO HealthNet (Medicaid)
- Must be under 21
- Must need at least 4 hours of skilled nursing per day
