Consumer Directed Services
ALLOWS A FAMILY TO TAKE CARE OF A LOVED ONE
Consumer Directed Services (CDS) is a Medicaid-based program that provides personal care assistant to you. The Consumer (client) has the comfort to hire and train the Attendant (caregiver) of their choose. Attendant will assist the consumer with Housekeeping, Laundry, Bathing, Showering, Meal Preparation, Essential Transportation and etc.
CLIENT QUALIFICATION
- Receive Medicaid
- Have a physical disability
- Able to self-direct
CAREGIVER QUALIFICATION
- At least 18 years old
- Pass background checks
- Able to read and write
Independent Living Waiver (ILW) Program
At Priority One Healthcare Services, we support adults with physical disabilities through Missouri’s Independent Living Waiver (ILW). This program is designed to keep you independent in your own home by offering you control over your care.
Who Qualifies:
- Adults aged 18 to 64
- Must have a physical disability and require a nursing facility level of care
- Must be capable of directing your own care or have someone to do so on your behalf
