GUIDE Program (Dementia Support Services)

At Priority One Healthcare Services, we proudly support individuals and families through the Medicare GUIDE Program—an innovative care model designed for those living with dementia.

The GUIDE Program (Guiding an Improved Dementia Experience) provides comprehensive, coordinated support at no cost to eligible Medicare beneficiaries. This program focuses on improving quality of life while helping individuals remain safely in their homes.



Through GUIDE, clients and their families receive personalized care planning, ongoing support, and access to a dedicated care team. Services include care coordination, caregiver education, 24/7 support access, and respite care to give families the relief they need.

Our goal is to ensure that both clients and caregivers feel supported, confident, and never alone while navigating the challenges of dementia care.

Who Qualifies

To be eligible, a person must:

• Have Medicare Parts A & B
• Have a diagnosis of dementia (Alzheimer’s or related conditions)
• Live at home or in assisted living (not a nursing home)
• NOT be enrolled in:
• Hospice
• PACE
• Certain Medicare Advantage plans (in some cases)
